I filled out a form to apply for the frequent flyer miles during a layover at the Dallas Fort Worth airport. My husband came over and said to not do it because we didn't fly often enough to bother with it. I stopped --I'm not sure I had completed the form, but maybe so because a few days later I found an "approval". I should have requested to be given the form but didn't think to do that. I was alerted by text message that there was a concern. When I got home I found an email and replied the details. I was advised to contact American Airlines and have the account closed. AA was helpful and put me through to the correct number. They will send me a proof of closure.
